Elevrae Health Membership provides structured, physician-led follow-up between annual Comprehensive Health Assessments, with repeat objective testing and lab work to track progress, additional screenings when indicated, appropriate medication management, and ongoing adjustments to your plan as your health and goals evolve.
QUARTERLY FOLLOW-UP
One-hour visits that include repeat vital signs and body composition scanning along with a physician visit to review progress and make personalized plan adjustments
FOLLOW-UP LABORATORY TESTS
Orders for selected, personalized repeat lab testing
PREVENTIVE SCREENING TESTS
Orders for indicated screenings including coronary calcium scan, DEXA (bone density) scan, breast imaging, and colonoscopy
MEDICAL MANAGEMENT
Selected medication prescriptions when clinically appropriate including medications that target cholesterol, blood pressure, blood sugar, weight, and female hormone replacement

Elevrae Health Membership is designed to complement—not replace—your primary care. Members receive structured follow-up focused on prevention, lifestyle medicine, health optimization, and select medication management within the scope of Elevrae Health.
For care outside that scope, we will coordinate with your primary care physician or refer you to the appropriate specialist.
